Latrobe Valley Chronic Disease Action Plan

The Latrobe Chronic Disease Action Plan (2019–2025), led by the Latrobe Health Assembly, brought organisations together to improve the experience of living with chronic disease in the Latrobe Valley.

The Plan emphasised strong collaboration, shared governance, and community engagement. Through coordinated action across early detection, self-management, and community-based care, the Plan supported more connected, person-centred services across the chronic disease continuum.

This collaborative approach strengthened relationships and enabled partners to work toward a shared goal. The partnerships and collaboration continue through partner organisations and the Chronic Disease Community of Practice led by Latrobe Regional Health and Latrobe Community Health Service.
